Body Mass Index (BMI) is a ratio of weight in kilograms divided by height in meters squared. This is the acceptable standard for classifying obesity. A BMI of 30 or greater meets criteria for some weight loss surgery procedures. With a BMI of 35 – 40, many insurance companies will look for at least one serious medical condition related to your weight. A BMI of 40 or greater is often enough to gain bariatric surgery coverage through your insurance. The major weight related, or comorbid, conditions that lead to bariatric surgery coverage are:
- Diabetes
- High blood pressure
- Sleep apnea
- Fatty liver
A BMI of 30 or greater meets clinical criteria for bariatric surgery.
A BMI of 35 – 40 must be associated with at least one serious medical condition like Diabetes, Sleep Apnea or High Blood Pressure for the sleeve of gastric bypass.
A BMI of 40+ does not typically require an associated medical condition for bariatric surgery insurance coverage.

Terms & Conditions:
- Payment is due in full 2 weeks prior to scheduled surgery date. (ACH must be paid 3 weeks due to processing time.) There are NO EXCEPTIONS to this rule. Please plan to avoid rescheduling surgery.
- BMI cannot be greater than 60 (≤60)
- BMI must stay above 30
- Dr. Craig Chang's office will disburse the funds as needed before the surger
- Payment options: Cash, cashier’s check, debit/credit cards
- Credit card transactions $500 or more will have a 4% fee
- Dr. Chang’s office does NOT accept money orders, personal checks, Care Credit or United Credit
